The extension schedules a desktop notification for every upcoming match, firing exactly 15 minutes before kick-off. The notification appears as a native system alert — no need to have a tab open — and shows:

Reminders are scheduled automatically when Chrome starts and re-scheduled whenever the extension is updated.
