REMS Automator eliminates the manual, time-consuming process of completing Spravato® (esketamine) REMS Patient Monitoring Forms after each treatment session. Designed for Spravato treatment centers using PracticeQ/IntakeQ for clinical documentation, this extension reads your completed treatment note and fills the required REMS form automatically — saving your team 5–10 minutes per patient per session.
WHY YOU NEED THIS
The Spravato REMS program requires a Patient Monitoring Form (PMF) to be submitted after every treatment session, capturing vitals, dose administered, monitoring duration, adverse events, and more. Completing this manually by switching between your EHR and the REMS portal, transcribing data field by field, is tedious, error-prone, and takes time away from patient care. REMS Automator solves this.
WHAT IT DOES
Auto Submit REMS Form
Open a completed, locked Spravato treatment note in PracticeQ and the REMS portal simultaneously, click GO, and the extension:

Finds the patient in REMS automatically (including first-name fallback for patients whose names differ between systems)
Checks for duplicate submissions — if a form already exists for that treatment date, it stops and alerts you before creating a duplicate
Fills all required fields: provider information, clinic details, treatment date, dose (56 mg or 84 mg), lot number, monitoring duration, vitals acceptability, SpO₂ readings, concomitant medications, and serious adverse event status
Flags any required fields left blank so you can review before submitting

REMS Audit
Run a compliance audit across your entire patient panel with one click. The extension:

Pulls all checked-in Spravato appointments from PracticeQ for the last 7 days or yesterday
Cross-references each appointment against submitted REMS forms
Produces a clear report showing which patients are missing forms, which are not yet enrolled in REMS, and which are fully compliant
Runs entirely in the background — you can keep working while it checks 30+ patients automatically
Supports separate audits for Woburn and Danvers locations
