About

Vaultus AI is a safety verification layer for AI-generated clinical SOAP notes. Before you sign, Vaultus reads the note.

The problem

AI scribes write fast. They also hallucinate — wrong medications, wrong doses, dangerous drug interactions, missing safety screens. The current standard of care is asking providers to carefully read every note before signing. That's not a safety system. That's a wish.

What Vaultus does

Vaultus monitors your clipboard for clinical note content and runs it through a library of hard-coded clinical rules derived from national guidelines — ADA, Beers Criteria, CDC opioid guidelines, ACC/AHA, USPSTF, and others. The safety check is deterministic. It doesn't guess. It doesn't interpret. It checks the note against the rule and returns a pass or a flag.

Every clean note is a timestamped safety attestation. Every catch is documented evidence that your review process works.

Vaultus was built by a practicing Family Nurse Practitioner who uses it on his own notes. The rules are sourced from national guidelines, not textbooks or interpretive references. Every flag is citable. Every pass is documented.

Your data

Clinical content is processed for safety verification and immediately discarded. De-identified catch metadata (not note content) is retained for software development and safety research. Vaultus does not store, sell, or transmit your patient notes.
