• Alt text over 250 chars flagged as suspicious
• Tracking pixels (1x1 or smaller) automatically excluded
Headings
• No H1 on page (critical)
• Multiple H1s on a single page (warning)
• Heading levels skipped (H1 → H3 without H2)
