    Paste or type any CARC code (CO, PR, OA, or PI) to get a plain-language explanation, the regulatory authority behind the denial, and a guided decision path: appeal, write off, or hold.
    
    GUIDED APPEAL WORKFLOW
    A short set of questions returns a specific recommended action for each code. The same logic powers the BillingBench Denial Decoder at billingbench.com/decoder.

    Select a specialty and enter a CPT code to see exactly what Aetna, UHC, or Cigna requires before they will authorize the procedure — documentation, clinical criteria, and submission notes — without leaving the portal.

    Version 1.2 introduces IRIS, a deterministic search interface over BillingBench's verified policy corpus. Typing a denial code, payer name, or plain-language query returns a structured answer drawn directly from the verified data: the regulatory argument, statutory closing demand, source citation, and a link that opens the relevant BillingBench tool with context pre-filled. Every IRIS result cites a primary source. Nothing is generated from language model inference. When the extension detects a recognized payer portal, the search placeholder surfaces the detected payer as a hint so queries can be targeted without retyping the payer name.
    
    835 ERA Detection
    When the extension detects an ERA download in the browser for accounts with an active Business Associate Agreement, a banner appears in the sidebar with a one-click route to the BillingBench 835 ERA Parser for immediate denied-claim analysis.

    The Calc panel covers two related calculations. The prompt pay section computes statutory interest owed on overdue claims under the applicable state statute for all 50 states and DC, producing the exact interest figure and citation for a demand letter. The timely filing section looks up the filing deadline by payer, calculates days remaining from a date of service, and flags claims approaching or past the window.
    
    Verified Payer Alerts
    The Alerts panel displays confirmed payer policy changes from BillingBench's editorial team: prior auth requirement changes, portal routing shifts, and denial pattern updates that affect billing workflow. When the extension detects a payer portal, alerts filter automatically to that payer.
    
    Community Intelligence
